Dear Patient,

To facilitate remote monitoring and to provide access to super specialty endocrine care to patients who are on follow-up and are unable to visit the clinic, our clinic has come up with a remote monitoring.

The procedure is as follows:

• Dr. Hemant Phatale 8459438215 with email marked to [email protected]

E Mail/WA should include following details:

• Name
• Registered and working Mobile number
• ID number of OPD
• Current medication list and last prescription

Since remote monitoring can be advised only under exceptional situations when patient cannot come to clinic mail should also include a note that "patient has asked for remote monitoring and has consented for remote treatment" Also remote monitoring should be followed up by a visit to clinic within 6-12 weeks of remote monitoring.
